To help you get settled, here are a few practical tips I share with my clients in getting all the paperwork done.
Items to request from the Landlord:
Go to the Barangay and Village Association and get the following forms
Applying for a Car Sticker
San Lorenzo Village Association (SLVA) ID (Resident’s ID)
The SLVA ID can be used for entry to the SLVA gates, use/rent of village facilities, car sticker application (for May), and more.
Steps in getting a SLVA ID
1) Fill out the ID form from the SLVA office
2) Bring a photocopy of a valid ID
3) Prepare for a photo to be taken
4) Pay the ID fee of P125
5) Processing time and Hours
Operating hours, Mon – Fri 8am-11:30am, 1pm-2pm; Sat 8am-11:30am.
ID can be printed immediately
Resident’s Certificate – SLVA
The SLVA Resident’s Certificate is required to get a Barangay Clearance.
Steps in Getting a Resident’s Certificate
1) Letter of request from tenant addressed to San Lorenzo Village Association – state request for certification, address, photocopy of tenant’s new SLVA ID, copy of contract (just to be sure)
2) Payment of P20
3) Submit to Ms Diane (SLVA)
4) Processing time: within the day if the village association officer is in the office. Otherwise, next business day.

Barangay Clearance
This is required for i-card application
Steps in getting a Barangay Clearance from Barangay San Lorenzo
1) Submit letter of request addressed to Barangay San Lorenzo, Clearance Dept. Look for Clearance, Ms Matet
2) Attachments needed:
a) Photocopy of tenant’s passport and SLVA ID
b) SLVA Certification
3) Pay Clearance fee of P100
4) Processing time: next business day

Telephone, Internet, and Cable
My SLV tenants and friends say that PLDT offers the most reliable Internet service in the village.
Steps in applying (with Existing Line)
1) Prepare the requirements
– Authorization letter – from Landlord letting Tenant use her plan (indicate full name, account number, landline, and complete billing address)
– Letter of Request – indicating the desired Internet/cable plan
– copies of ID of Landlord and Tenant
2) Email to homeinboundcso@pldt.com.ph
3) Call the PLDT hotline -888-8171 – New application to followup and request for service order. Be sure to get 2 service orders if you plan to apply for Internet and Cable plan.
4) Tech team to go to the site within 6 calendar days upon job order.
Steps in applying (New subscription)
- Prepare the subscription details
- Full Name (First Name,Middle Name, Last Name) (Required):
- Birth Date (Required):
- Civil Status (Mandatory):
- Valid ID w/ number (Provide Soft Copy of ID or path where we can get the ID) (Required):
- Home Ownership (Mandatory):

- Plan Request (Required):
- Complete Address (Required):
2. Email the subscription details to customercare@pldt.com
3. Call the PLDT hotline -888-8171 – New application to followup and request for service order.
Sky Cable

Steps in Applying:
1) Text Imelda (a Skycable agent) – 0905-3874963 the ff info
– Name of Subscriber (tenant)
– Address
– Plan
2) Their tech team will inspect the location and will bring a complete subscription form
3) Installation will be done a few days after inspection.
